At this session we will explore chronic disease trends, focus in on some not so well recognized chronic diseases, and explore chronic disease prevention, treatment and management. Experts will discuss the impact of chronic diseases on patients and on benefit plans. We will also explore options to proactively work with patients to detect these conditions early and manage them for optimal health outcomes.

This will be a great opportunity to explore the impact that plan design, education, wellbeing and disease management initiatives can have on chronic disease management and benefit plan experience.
